    National Sickle Cell Anemia Elemination Programme Disease Control Programme

    • Sickle Cell anemia is a genetical disorder in which the red blood cells can become sickle shaped due to abnormal sickle Hemoglobin (that is, shaped like a “C”). Normal red blood cells are smooth and round like a doughnut without a hole and they move easily through blood vessels to carry oxygen to all parts of the body while Sickle-shaped cells do not move easily through blood; they are stiff and sticky, hard and tend to form clumps and get stuck in blood vessels. The clumps of sickle cells block blood flow in the blood vessels to the limbs and organs. Blocked blood vessels can cause pain, serious infections and organ damage (pain crisis & respiratory infections)
    • Sickle cell is more prevalent in tribal population as compared to general population. Sickle cell disease has no curative treatment.
    • Types of Sickle cell:
    1. Sickle Cell Trait/Carrier – Sickle Hemoglobin (HbS) less than 40% (These persons are quite healthy but responsible for disease transmission in next generation)
    2. Sickle Cell Disease/Sufferer – HbS more than 40% and patients are at risk of bacterial infection, joint pain and vital organ failure.

    Government of India (GOI) has launched National Sickle Cell Elimination Mission on
    1 st July 2023 under which mass screening has to be done in 21 Districts Maharashtra
    including all tribal districts like Thane , Nashik , Nandurbar , Amravati , Gondia, Gadchiroli,
    Palghar , Nagpur, Wardha , Chandrapur , Bhandara, Yavatmal , Dhule, Jalgaon , Nanded,
    Buldhana , Washim, Akola, Aurangabad , Raigad & Hingoli

    • with target of elimination of disease till 2047 & given Sickle Cell screening Target of 63 Lakhs for year 2023-25 of 0 to 40 year age group. Till date 16/01/2025 state have achieved screen target of 53Lakhs (82%)
    • GOI has also launched a sickle cell Web portal and Mobile App on which the screening data should be entered by each district. All the entries are uploaded on this portal.
    • District Sickle Cell Co-ordinator, Sickle Cell Counsellor, Laboratory Technician, Taluka Sickle Cell Program Assistant, Counsellor are manpower approved for screening, treatment and awareness in the districts.

    Objectives Of The Program:

    1. Sickle Cell Screening facilities at PHC, RH, WH & DH by Solubility testing.
    2. Confirmation of Sufferer / Carrier by HPLC testing
    3. Improvement of Quality of life of diseased by regular treatment (Hydroxyurea & Blood transfusion as per need)
    4. Marriage-Counselling of carrier and Sufferer by Genetic Card
    5. Awareness and information regarding Sickle Cell Disease transmission in next generation & its prevention.
    6. Prenatal diagnosis of targeted ANCS to avoid new birth of sickle cell disease.

    Services Provided Under Sickle Cell Disease Elimination Control Program:

    • Free Solubility testing as screening test.
    • Free HPLC Test for confirmation of Disease.
    • Counseling( Transmission of disease, prevention, diagnosis & management)
    • Prophylactic & Symptomatic treatment to be provided(like Hydroxyurea & Folic Acid )
    • Regular Health checkup for patients on Hydroxyurea (CBC, LFT, KFT) & follow up of Sufferer.
    • Free Blood transfusion to Sickle cell Sufferer as per need
    • Under Sanjay Gandhi Niradhar Yojna GR dated 03 Nov 2018 Rs. 1000/- monthly will be given to sickle cell patients as support.
    • Sickle cell disease is included in MJPJAY/PMJAY for management.
    • IEC in the form of Day celebration on 19th June and sickle cell week celebration from 11th Dec to 17th Dec per year in all 21 districts are conducted.
    Disability certificate received to sickle cell patients to district level.

    Hemophilia

    • Hemophilia is a serious genetic disease in which there is a deficiency of blood clotting factors.
    • There are approximately 5,000 hemophilia patients in the state at present.
    • For the treatment of these patients, one (total 36) day-care centers have been set up in all the districts.
    • Anti-Hemophilic Factors (Factor VII, VIII, IX, FEIBA) are used for the treatment of these patients.

    Thalassemia

    • Thalassemia disease is a serious genetic disease in which the amount of hemoglobin produced in the blood is very low, due to which the hemoglobin level of these patients is very low.
    • These patients require regular blood transfusions.
    • Free blood is provided to these patients from all the blood banks in the state (a total of 393 blood banks).
    • At present, there are approximately 10,000 patients with this disease in the state.
    • These patients require regular iron-reducing drugs.
    • Regular blood transfusions are done to these patients in the district hospital / medical college.
