    Introduction-

    In most developing countries, provision of basic preventive, promotive and curative services is a major concern of the Government and decision makers. With growing population and advancement in the medical technology and increasing expectation of the people especially for quality curative care, it has now become imperative to provide quality health care services through the established institutions.

    Upgradation of CHCs to Indian Public Health Standards (IPHS) is a major strategic intervention under the National Rural Health Mission (NHM). The purpose is to provide sustainable quality care with accountability and peoples participation along with total transparency.

    Concept-

    Rogi Kalyan Samiti (Patient Welfare Committee) / Hospital Management Society is a simple yet effective management structure. This committee, which would be a registered society, acts as a group of trustees for the hospitals to manage the affairs of the hospital. It consists of members from local Panchayati Raj Institutions (PRIs), NGOs, local elected representatives and officials from Government sector who are responsible for proper functioning and management of the hospital / Community Health Centre / FRUs. RKS / HMS is free to prescribe, generate and use the funds with it as per its best judgement for smooth functioning and maintaining the quality of services.

    Objective of RKS :-

    • To improve the health services provided by hospital & its outreach fields.
    • To monitor the process of implementation of National Health Program.
    • To organize outreach services/ health camps at facilities under the jurisdiction of the health institution.
    • To ensure that Government Health services are provided as per minimum requirement and to understand the response of beneficiaries regarding.
    • To support hospital in terms of donation or other financial activities.
    • To utilize the available fund by decentralization method.
    • To do all needful procedures for patients to make available sufficient qualitative servicesrequired.

    Objectives/Functions and Activities of RKS:-

    1. Undertake all activities to improve and expand patient related quality services  and welfare activities.
    2. Acquiring equipment, furniture, emergency  medicines for efficient running of the hospitals as per existing rules and regulations.
    3. To ensure subsidized food, medicines and quality drinking water to the patients and their relatives.
    4. Improve boarding and lodging arrangements for patients and there attendant.
    5. To put a management  structure in place that could fulfill the objective of community participation.
    6. The main objective of RKS is to decentralize the hospital functioning by delegating the financial powers to the head of the institutions.
    7. The  Executive Committee will guide and control the activities as per the policy formulated by RKS Governing Body .

    • Uses of RKS, AMG, and UF Fund are as Follows:-

    RKS Fund is utilized for:-

    • Purchase of retail medicines for poor & Needy patients.
    • Laboratory tests outside hospital
    • Providing referral services to Poor Patients.
    • Provision clothes to poor patients and pregnant women/newborn.
    • Provision of Drinking water facility to patients
    • Sitting arrangement in waiting room for patients
    • Install Solar power when hot water is available for in patients
    • Miner Repairs and Maintenance for Kitchen / Hospice.
    • Arranging food for poor patients.
    • Purchase of essential equipment if not available in the Hospital
    • Other innovative schemes.

    AMG Fund is utilized for:-

    • Cleaning and Maintaining a toilets of hospitals.
    • Repairs & maintenance of Septic tack.
    • Proper disposable of sewerage by repairing & drainage
    • Cleaning the drinking water tank, and fixing water supply line.
    • Repair the wash Basins.
    • Provision of basins for hand washing.
    • Cleanliness of washrooms, by making necessary repairs to the building.
    • Repairs of furniture / doors/ windows (Furniture works)
    • Destruction of mosquito breeding sits
    • Keeping premises clean.

    UF Fund is utilized for:-

    • Purchase of medicine which is urgently required & if not available
    • Purchase of furniture/ curtains/ mattresses.
    • Provide referral services to the patients in case of emergency.
    • Purchase of lab materials.
    • Implementation of Innovative schemes to enhance the quality for the hospitals
    • Purchase of Furniture/ Curtains/ beds/ dressing Materials etc.
    • Spending for Xeroxing and Stationary

    Some Important

    • RKS is a registered body under Society Registration Act 1860 & Mumbai public trust Act 1950.
    • Delegated powers to RKS & District Health Societies to plan and implement activities as per their needs.
    • Planning and monitoring process of RKS should be through Governing body & Executive committee decisions
    • RKS can raise money through Donations & Bank Interest.
    • RKS is expected to facilitate better service delivery in the institutions.
    • Any change in the membership of Governing and Executive Committee members could be done only with the prior approval of Charity Commissioner‘s Office.
    • Annual Maintenance Grants are distributed for maintenance of physical Infrastructure of health institutions.
    • GOI has directed to use the Annual Maintenance grants in that institute which are Functional in their own building.
    • Major part of the Annual Maintenance grant should be spent to do minor repairs and strengthening of the building and doing cleanliness activities.
    • Untied funds have been provided to the institutions up to Village health nutrition sanitation& water supply Committees, Sub Centers, PHCs, CHCs, and PHU to promote local health action.
    • Necessity of Untied Fund has been felt mainly due to unavailability of funds for undertaking any innovative need based activity.

    JAN AAROGYA SAMITI

    • GOI Guidelines committee at the Sub Center level and Primary Health Center for AB-HWC.
    • Guidelines have been sent for implementation at district level.
    • G.R. Published by GOM dated on 23 Nov, 2022 Regarding the establishment & implementation of Ayushman Bharat Yojana Jan Arogya Samittee in Sub Centers and Primary Health Centers under National Health Mission.

    Village Health Nutrition Sanitation Committee
    Objective of the VHNSC

    • To provide an institutional mechanism of the community to be unformatted of health
      programmers and government initiatives and to participate in the planning and implementation
      of these programmers, leading to better outcomes.
    • To provide platform for convergent action on social determinants and all public service direct
      to indirect related to health
    • To provide an institutional mechanism for the community to voice health needs
    • To empower panchayats with the with the understanding and mechanisms required for them to
      play their role.
    • To provide support and facilitation to the community health workers.

    • Nutritional schemes for malnourished children
    • Transporting pregnant women for delivery if not available for government vehicle
    • Transporting other patients for treatment
    • Organization of Health Awareness activities
    • Purchase of TCL for safe water supply (in Kilo)
    • Repairs of water supply scheme

    Uses of RKS, AMG, and UF & VHNSC Fund are as Follows:-

    Purchase of medicine to poor patients, laboratory test, Referral transportation for poor patients, Food facility for patients, Cloths to patients/ newborn for delivery patients, purchase of essential equipment if not available in the Hospital, Repairs & maintenance of hospital building & quarters, Repairs & maintenance of water supply & drainage, Repairs of furniture / doors/ windows/ instrument / equipment/ delivery table etc. Purchase of dressing material (bandage/ cotton), Transport of women in labor, other innovative schemes, Minor Civil works, Maintenance of computers & other office equipment & other maintenance expenses.
